The problem is that NP's and PA's aren't able and aren't going to want to pick up the slack. The primary care shortage is much too large for even the growing number of mid-levels to abate. And NP's and PA's follow the money, as well. PA's are already leaving primary care in droves. KevinMD has a nice summary here:
http://www.kevinmd.com/blog/2008/12/why-nurse-practitioners-and-physician.html

I'm all for restructuring the health care system. But the primary care shortage is here now, and it needs aggressive action if it's to be kept from getting worse.

Hmm, I don't think this kind of spending is going to be a long-term fix for the problem. Paying people to do the right thing doesn't have a very good track record in promoting long-term behavioral change ($20 for your A in history class?). NPs and PAs can pick up a lot of the slack in family medicine, and their training is shorter and cheaper. It's more important that we restructure the health care system to make it more cost-effective. That might end up making gross doctor pay go down, which no physicians want to see. If you look at other countries with better health care systems, doctors are paid a lot less than they are here. But they also have to put up with a lot less inefficiency and litigation and their education is often heavily subsidized by the state or even free.

Amen. I'm thinking about doing an internship with Wired.com either this winter or the spring. By the way, you should read Not Exactly Rocket Science http://scienceblogs.com/notrocketscience/ if you don't already. It's by Ed Yong :)

Hi Ed,

I am going to share this article with Andrew and Greg. Although between the two, they consume 1, 2L bottle of a soda every week, they consume snacks that contanin HFCS. One more reason to make a dinner every day from fresh ingredients and sit together with everyone at least once per day to catch up.!

Well, I am going to share this article with my students who come from poor neighborhood and have very poor eating habits. I see kids bringing a 1/2 gallon - gallon sweetened ice tea every day and they really drink it all before the bell rings at 2:10 pm. It is norm among my students that they live with one parent who holds 2 jobs to make ends meet or foster parents with 10 or so other kids. Do they get to eat freshly made food? I highly doubt it.

Also, I have an added concern about HFCS. What about HFCS made in China, a country known to have outrageousely poor regulations and produces large quantities of just about anything the world consumes? (Remember the baby formula with melamine, lead content in toys, and fish with unapproved antibiotics? ). I never buy any fish farmed in China but I am at a loss when it comes to processed food stuffs since they do not list where the ingredients are come from.

I am so glad the planting season is here. Cold crops went in this weekend - organic and delicious!

Love you and say hi to Carisa
ps: Hope this summer, we get to see you. Sandra signed up for the summer week in Ashville. Yeah!

Everyone wants the results, no one wants to do the work. I think jury duty would drive me nuts. I can't handle sitting idle for that long and I'm sure they wouldn't let me bring my knitting.

Oh good grief that nighttime exposure is beautiful! Well done big bro. I love all your Middle East photos; they make me really want to go to places where there are thousands-of-years-old things. I know you were thinking about Indiana Jones when you took the middle pic.

So,
That was the reason everyone was jumping. All I could think of was. Hmmm, these guys must have gone to a low gravity place (not as low as the Moon). but then I could not explain the large table, an unlikely object for a simulated spaceship. Well, jolie jolie.

I get questions from my students whether I am one for evolution or for the other. My answer to that is, always, "In my personal opinion, there is room for both." Even as most people think scientists are all so logical and fact-based etc., I can't help but to think how intricate and well-designed all living things are. Well even the non-living things. I guess I see and know a bit more than many people who do not have much background in chemistry, a science about seeing what can't be seen.

Yeah! when is that pay raise coming for the poor teachers!
